Who we are. Continuum is a recovery coordination platform operated by ContinuumRTW Inc. ("Continuum", "we"). Continuum helps injured workers, their employer, their clinic, and the workers compensation board stay coordinated from injury to return to work.
The most important thing to understand. Continuum is not your medical record and not your doctor. Your clinic remains the custodian of your health information. Continuum manages information as a service provider to your clinic and your employer under written agreements, and under Canadian privacy law including the Personal Information Protection and Electronic Documents Act and, in Alberta, the Health Information Act and the Personal Information Protection Act.
What we collect. 1. Contact and identity: your name, phone number, and job title. 2. Injury and recovery information: the body part and type of injury, work restrictions, your pain and movement check-ins, notes you choose to write, your light duty tasks, and recovery milestones. Your clinic provides the clinical details when your case starts. 3. Employment information: your employer, your role, and dates connected to your claim. 4. Claim documentation: the information required for workers compensation forms, which can include your social insurance number provided by your clinic at intake. You are never asked to type it into the app. 5. Technical basics: device type and app version, so the app works and stays secure. We do not collect your location. We do not use advertising or tracking technologies. There are no third-party analytics in the app.
Why we collect it. Only to run your recovery: reminding you to check in, showing your care team how you are doing, matching light duties to your restrictions, alerting your clinician if your recovery needs attention, and preparing the workers compensation documents the law requires. We do not sell personal information. We do not use it for advertising. We do not use your health information to train artificial intelligence systems.
Who sees what. This is the heart of Continuum: 1. You see your own information. 2. Your clinician at the clinic sees everything about your case, so they can care for you. 3. Your site safety contact sees your scores and restrictions so duties stay safe, and never your diagnosis or your notes. 4. Your employer sees your work status, restrictions, and return progress, and never your pain scores, your diagnosis, or your notes. 5. The workers compensation board receives the milestone documents the law requires. These limits are enforced in the software itself, not just by policy.
Consent. We collect your information only after you agree in the app, and you can withdraw that agreement at any time in the privacy settings. Withdrawal stops collection immediately. Some information already connected to your claim must be kept by law even after you withdraw.
Where your information lives. In Canada. Our databases and file storage run in a Canadian region, encrypted in transit and at rest. Our service providers are listed below and are bound by contract to protect your information: Supabase (Canadian-region database and storage), Twilio (text messages; messages never contain health information), Vercel (delivers the web dashboards; does not store your health information).
How long we keep it. For as long as your clinic and the law require for a workers compensation matter, and no longer. Records are then destroyed and the destruction is logged.
Your rights. You can ask to see the information we hold about you and receive a copy from within the app. You can ask us to correct something; corrections to health information are reviewed with your clinic and the original is annotated, never silently rewritten. You can withdraw consent. You can complain to us, to the Office of the Privacy Commissioner of Canada, or to your provincial Information and Privacy Commissioner.
If something goes wrong. If a privacy breach creates a real risk of significant harm to you, we will notify you, the responsible custodian, and the required regulators, and we keep records of every breach.
